Dream Team Space Reservation

If your Dream Team needs to reserve space for upcoming meetings or events, please use this form to submit your request.

Facilities Reservations

If you would like to reserve the Life Center for a private party or event, use this form. *Note if you need to reserve our facilities for your Central Group or Dream Team, use the Dream Team Space Reservation form instead.

IMPACT Event Proposal

If you have an idea for an IMPACT event, tell us about it here. One of our IMPACT organizers will get in touch with you to discuss it further.

After your Central Group or IMPACT event, use this form to tell us what happened. We love to hear testimonies of the Lord moving on people’s lives and we also want to be able to track progress and participation.
